COURSE: EFFECTIVE ACCIDENT INVESTIGATION AND REPORTING
COURSE CODE: AIR 101

COURSE INTRODUCTION
This course introduces you to basic accident investigation procedures and describes accident analysis techniques. Throughout the course, you’ll be taking what you’ve learned throughout the course to analyze a hypothetical accident!
The challenge to any accident investigator is to report the findings in a well-thought-out manner to ensure management will adopt recommendations for improving its safety management system, thus solving problems long-term. It’s a common struggle trying to overcome long-held perceptions about safety and how accidents occur.

COURSE OBJECTIVES
By the end of the course delegates will be:
 Familiar with the relevant regulations and the reporting requirements of those regulations.
 Aware of the correct methods of investigating an accident/incident thoroughly and effectively.
 Able to gather evidence and interpret findings logically and methodically.
 Aware of the need for efficient evidence collection and the correct methods of storing and protecting evidence.
 Capable of completing samples of relevant documentation.
 Able to interview and take statements from witnesses.
 Familiar with the basic causes of accidents and incidents.

Module One: Initiating the Investigation
Module Two: Documenting the Accident Scene
Module Three: Conducting Effective Interviews
Module Four: Conducting Event Analysis
Module Five: Determining Surface and Root Causes
Module Six: Developing Effective Recommendations
Module Seven: Selling Safety to Management
Module Eight: Writing the Report

Target Audience
All personnel who have a responsibility for health and safety in their organisation and may be required to investigate accidents and incidents and to report them.

DURATION
One Day

Methodology:
Class room session,
Course manual for all participants
Digital in-focus projector to display lecture slides
At the end of the course, participants shall be issued certificates.

Revalidation:
Every three years

Health:
All delegates will be asked to complete a medical questionnaire prior to course commencement.
